Wide view surgical management of vitreoretinal pathologies help in better understanding of the diseases. Conversion from 20 gauge to 23, 25 and 27 gauge Minimally Invasive Vitreous Surgery was for better postoperative comfort and better surgical outcome. Surgical outcomes can be much improved with wide angle viewing system with good magnification and  illumination.  The modern trends with MIVS are highlighted to deal different vitreoretinal pathologies.
